Please click HERE to read and access the online immunization exemption module. You will need to watch the module and have the certificate sent to your email and then print the certificate and bring it into the office for us to copy for your student's file.
If you choose to be exempt from immunizations, you are REQUIRED to do this module and gain the certificate and we MUST have a copy in the student's permanent folder here at school.
